The Virginia Asthma Action Plan is a guide used to aid families and schools on how to manage a child’s asthma and completed by the health care provider managing the child’s asthma. The Virginia Action Plan is revised every 2 years. The Virginia Asthma Action Plan Expert Group convened by the Consortium for Infant and Child Health at Eastern Virginia Medical School oversees the updates and release of the plan. This form is the approved form for use across Virginia.
